Martin Holy, with registered address at Sramkova 164, 460 10 Liberec XX-Ostasov, Czech Republic, company ID 08191131, operates Forms7.com, available at https://www.forms7.com. Forms7 enables registered users to create, publish, manage, and process online forms under these terms.

By creating an account or using Forms7, you agree to these terms.

Forms7 provides tools for creating online forms, collecting submissions, managing form content, exporting data, connecting integrations, accepting payments where available, and analyzing submission data. Some features use automated and AI-assisted systems to generate suggestions, drafts, summaries, classifications, logic, or insights.

Public forms may be made available through Forms7-hosted URLs, embeds, or customer websites, depending on the publishing method selected by the user.

Only registered users may create and manage forms in Forms7. You are responsible for keeping your account secure and for all activity under your account.

You must provide accurate account and billing information and keep it up to date.

We may suspend or terminate access to Forms7 if you violate these terms, fail to pay amounts due, create legal, security, operational, or reputational risk, abuse the service, harm other users or third parties, or if suspension or termination is needed to protect Forms7, comply with law, or preserve the service.

You are responsible for the forms, submissions, files, text, images, and other content that you create, upload, collect, or process through Forms7. You must ensure that your content and use of Forms7 comply with applicable law, these terms, third-party rights, public order, and good morals.

Forms7 is not responsible for user content. We may remove forms or content that violate these terms or applicable law, including content that infringes intellectual property rights or is used to store large files outside the normal purpose of the service.

You are responsible for reviewing AI-assisted output before publishing or using it. You must not rely on AI-assisted output as professional legal, financial, medical, tax, or other regulated advice.

When you upload or publish content through Forms7, you grant us a non-exclusive, royalty-free license to use that content only as needed to provide, secure, maintain, and improve the service, including AI-assisted features you choose to use, during the period in which the content is available through Forms7.

You may remove your forms and content at any time, subject to technical processing, backup, legal, and billing retention requirements.

We work to keep Forms7 available and secure, but we do not guarantee uninterrupted operation. We are not liable for loss of files, information, submissions, service availability, or form functionality caused by technical problems, outages, maintenance, third-party services, or events outside our reasonable control.

Third-party APIs, integrations, payment providers, authentication providers, AI providers, and external platforms may change, suspend, limit, revoke, or discontinue their services. Forms7 is not responsible for failures, delays, data loss, or feature limitations caused by those third-party services.

Paid plans are billed according to the pricing shown at the time of purchase or renewal. We may change pricing for future billing periods.

Recurring plans are paid automatically using the selected payment method. Payments cover the following billing period. If a monthly plan is cancelled, already started billing periods are not refunded unless required by law. Access continues until the end of the paid period.

If payment is overdue, we may suspend or terminate access to paid features.

We may remove forms or accounts that have not used the service for more than six months. Where reasonably possible, we will notify the user by email after five months of inactivity and provide one month to restore activity.

We process personal data in accordance with the GDPR and other applicable data protection laws. Personal data needed to provide Forms7 is processed because it is necessary to perform the contract with the user. Additional processing may occur where required by law, based on legitimate interests, or based on consent.

If you submit personal data of other individuals through Forms7, you are responsible for informing them and ensuring that you have a valid legal basis for the processing.

We may update these terms in a reasonable scope, especially due to legal, technical, product, operational, or business changes. We will notify users through the service or by email before material changes take effect. If you continue using Forms7 after the effective date, you accept the updated terms.

These terms are governed by the laws of the Czech Republic, unless mandatory consumer protection laws provide otherwise.
